If your business qualifies for tax exemption, please download and complete the Wisconsin Sales and Use Tax Exemption Certificate. Once completed and signed, submit a copy of this form to our office. This form needs to be on file at our office in order for the sales tax to be exempt from your account. Forms are also available at our office.
